Use this form to officially enroll your church in the Global Alliance Strategic Communities’ Square Mile Partnership program.

You will provide:

2. Areas of interest (e.g., digital tools, outreach funding)

3.Key partnership goals

BUSINESS ENROLLMENT FORM

Register your business with Global Alliance to access digital tools, support, and our Black-owned business directory used by partnering churches.

You’ll provide:

1. Business type and niche

2. Contact details

3. Services you're interested in

4.Your short- and long-term business goals

CONSENT & DATA PROTECTION FORM

This form confirms your agreement with how Global Alliance collects and uses your data in compliance with privacy regulations. You’ll confirm:

1.The data collected

2. Purpose of use

3. Your formal consent

REFERRAL FORM (CHURCH/BUSINESS)

Use this form to refer churches or Black-owned businesses to join the Global Alliance network.

You can refer:

Up to 12 individuals or organizations

As a pastor, business owner, or coordinator

FUNDRAISING SUPPORT FORM (CHURCHES ONLY)

This form allows churches to apply for financial support for specific campaigns or initiatives.

You’ll provide:

1. Fundraising goal and target amount

2.Donation records

3. Bank or M-Pesa details for transfers
